29 CFR  1926.651(c)(2):A means of egress from trench excavations. A stairway, ladder, ramp or other safe means of egress was not located in trench excavations more than 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees.  On or about February 7, 2022, two employees were in an excavation which was up to approximately eleven feet deep to expose damaged electrical conduit lines without having a ladder present, exposing workers to a cave-in hazard.

29 CFR  1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c).  On or about February 7, 2022, two employees were inside an excavation that was up to approximately 11 feet deep without the use of a protective system, exposing employees to a cave-in hazard.

29 CFR  1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person prior to the start of work, and as needed throughout the shift, for evidence of situations that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions.  On February 7, 2022, two employees were removing soil to expose electrical conduit lines in the bottom of a trench up to approximately 11 feet deep without the competent person conducting an inspection, exposing employees to a cave-in hazard.
