29 CFR  1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a) At the jobsite, an employee on the rails of the SkyJack SJIII 3219 Scissor Lift, approximately 12 feet above the ground, was not protected from falling.

29 CFR 1926.453(b)(2)(v):  A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a) At the jobsite, an employee conducting construction work from inside the basket of a Genie S-45 aerial lift did not have his lanyard connected to the basket, exposing the employee to a fall hazard of approximately 25 feet.
