Defective gear shall be removed and repaired or replaced before further use.  a) On 3/2/2022 the employer did not properly inspect the welds that was attached on 3/1/2022 that was a load bearing pad eye to the bottom of a vessel that employees utilized in the process of replacing a wheel on the bottom of a vessel. The welds failed during the lifting process.
