29 CFR  1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:   a) 11 Maple Ave Roof - On or about January 26, 2022 and at times prior thereto, employees were using a Lull rough terrain fork lift to support a scaffold platform that was not specifically designed by the manufacturer.

29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:   a) 11 Maple Ave Roof - On or about January 26, 2022 and at times prior thereto, employees were installing a 4 on 12 sloped metal roof without the use of fall protection.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:   a) 11 Maple Ave Roof - On or about January 26, 2022 and at times prior thereto, employees were installing a 4 on 12 sloped metal roof without the use of fall protection.  The employees stated they had received fall protection training but the employer did not provide training certification for such.
