29 CFR  1910.134(f)(2): Employee(s) using  tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator or annually refitted.    At Lakeland Nursing and Rehab, LLC located at 3680 Lakeland Lane, Jackson, MS 39216:  (a) On or about March 16, 2022,  the employer did not provide an initial or annual fit test to all employees. The employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed COVID-19 patients.

29 CFR  1910.502(q)(2)(ii)(A):The COVID-19 log did not contain, one form of contact information, occupation, location where the employee worked, the date of the employee's last day at the workplace, and the date the employee first had one or more COVID-19 symptoms, if any were experienced.  At Lakeland Nursing and Rehab, LLC located at 3680 Lakeland Lane, Jackson, MS 39216:  a.) On or about March 16, 2022, the employer failed to ensure the COVID-19 log contained, one form of contact information, occupation, location where the employee worked,  the date of the employee's last day at the workplace, and the date the employee first had COVID-19 symptoms.
