Permanent Floating Dock - On or about March 20, 2022, the employer exposed employees to a struck by hazard while loading an excavator/material handler onto a barge.

Permanent Floating Dock - On or about March 20, 2022, a compressed gas cylinder located next to a welding cart did not have a valve cap and the cylinder was not secured.
