29 CFR  1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets.   a- 253 E Crescent Ave, Mahwah NJ : CSHO observed that an employee was exposed to overhead impact  hazards, when an articulating knuckle boom crane using a fork attachment boomed pallets of shingles and delivered overhead to the employees on the roof as head protection was not utilized.  Condition observed, on or about 3/18/2022.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a- (6 on 12 pitch roof): CSHO observed  employees engaged in roof material staging activities and positioned on a 6 on 12 pitch roof exposed to fall hazards of approximately 16ft to the asphalt below as a means of fall protection was not being utilized. Condition observed on or about 03/18/2022
