29 CFR  1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece was used, and at least annually thereafter:   a) Shenandoah Heights Healthcare LLC- The employer did not ensure fit testing was conducted whenever a different respirator was used for employees required to wear respirators, on or about March 25, 2022.    Abatement certification required.  The employer, Shenandoah Heights Healthcare LLC, was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA inspection number 1510852, citation number 01, item number 01c, and was affirmed as a final order on June 28, 2021.
