29 CFR  1910.119(j)(4)(iii):The frequency of inspections and tests of process equipment to maintain its mechanical integrity, was not consistent with applicable manufacturer's recommendations and good engineering practices, or more frequently determined to be necessary by prior operating experience:  a)  Exxon Mobil Corporation Billings Refinery, CRF-103 Control Loop, Crude Unit Overhead System Hydrofiner 2 Feed:  On or about March 26, 2022 and at times prior, employees were exposed to fire and subsequent explosion created by a loss of containment of unstabilized naptha in the CRF-103 control loop circuit.  Existing populations of CMGC gaskets were not identified and removed in CL300 piping systems in accordance with ExxonMobil Corporation's own organizational learnings that small leaks from CMGC gaskets in CL300 piping systems could rapidly escalate into larger leaks.
