29 CFR  1910.147(c)(7)(i)(A): The employee did not ensure authorized employees received training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.   On or about January 11, 2022, the employer did not ensure that authorized employees were trained in the methods and means necessary for energy isolation and control on equipment, such as but not limited to Press #31, a die cutter press with KTI Turret rewinder model JR1312-05, Serial # 2513.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(c)(6)(i):The employer failed to conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.   On or about January 11, 2022, The employer failed to conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of 29 CFR  1910.147 are being followed in regards (but not limited) to Press #31, a die cutter press with KTI Turret rewinder model JR1312-05, Serial # 2513.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(d)(3):Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).  On or about January 11, 2022, the employer did not ensure that energy isolating devices were operated to isolate machines, including Press #31, from hazardous energy sources. Employee(s) were exposed to crushing and amputation hazards while accessing the hazard areas of the machine to perform work such as roller changes and cleaning to Press #31, located on the production floor.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
