29 CFR  1910.132(d)(1): The employer did not adequately assess and have each employee wear personal protective equipment (PPE) exposing employees to physical injuries. Instances include, but are not limited to the following:  WORKSHOP/PRESS BRAKE DEPARTMENT:  The employer had not conducted a workplace hazard assessment for personal protective equipment (PPE) such as (but not limited to ) cut resistant gloves for the operators in the press brake department.  On/or about March 22, 2022, an employee loading metal parts in and out of the press brake #7, the metal part hit the employee's resulting in a finger amputation.  ABATED DURING THE INSPECTION.

29 CFR  1910.132(d)(2):The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identified the workplace evaluated;  the person certifying that the evaluation has been performed;  the date(s) of the hazard assessment;  and which identifies the document as a certification of hazard assessment:  WORKSHOP/PRESS BRAKE DEPARTMENT:  The employer had not verified through a written certification that the workplace hazard assessment for personal protective equipment (PPE) had been performed as complete.  ABATED DURING THE INSPECTION.

29 CFR  1904.39(a)(2):The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.   ESTBLISHMENT:  The employer had not reported to  Federal OSHA within 24 hours the in-patient hospitalization of the injured/employee resulting from an amputation while operating press brake #7.  ABATED DURING THE INSPECTION.
