29 CFR  1910.134(k)(6):The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  a) 912 Salida Way, Aurora CO 80011:  On or about April 8, 2022, and at times prior, the employer did not provide the information contained in Appendix D of the respiratory protection standard to employees who voluntarily wear respirators.  Employees may voluntarily wear N95 filtering face-piece devices when cutting, grinding, or polishing stone slabs.  Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR  1910.1053(d)(1):The employer did not assess the exposure of each employee who was or may reasonably be expected to be exposed to respirable crystalline silica at or above the action level in accordance with either the performance option in paragraph (d)(2) or the scheduled monitoring option in paragraph (d)(3) of this section:  a) 912 Salida Way, Aurora CO 80011:  On or about April 8, 2022, and at times prior, the employer did not perform exposure assessment of each employee who may be reasonably exposed to respirable crystalline silica above the Action Level.  Employees are cutting, grinding, and polishing granite, marble, and synthetic stone slabs while fabricating and installing countertops.  Abatement Note: Abatement certification is required for this item (see enclosed Certificate of Corrective Action Worksheet).
