29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: a) 912 Salida Way, Aurora CO 80011: On or about May 3, 2023, and at times prior, the employer did not provide the information contained in Appendix D of the respiratory protection standard to employees who voluntarily wear N95 filtering facepiece respirators. The employer, Amazing Granite LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(k)(6), which was contained in OSHA inspection number 1589625, citation number 1, item number 1, and was affirmed as a final order on June 8, 2022, with respect to a workplace located at 912 Salida Way, Aurora, CO 80011. Employees cut stone slabs while fabricating and installing countertops. Employees preparing slabs were exposed to levels of respirable crystalline silica above the action level of 25 micrograms per cubic meter of air.
