29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  a) 912 Salida Way, Aurora CO 80011: On or about May 3, 2023, and at times prior, the employer did not provide the information contained in Appendix D of the respiratory protection standard to employees who voluntarily wear N95 filtering facepiece respirators.   The employer, Amazing Granite LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(k)(6), which was contained in OSHA inspection number 1589625, citation number 1, item number 1, and was affirmed as a final order on June 8, 2022, with respect to a workplace located at 912 Salida Way, Aurora, CO 80011.  The employer, Amazing Granite LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(k)(6), which was contained in OSHA inspection number 1619313, citation number 1, item number 1, and was affirmed as a final order on October 26, 2022, with respect to a workplace located at 912 Salida Way, Aurora, CO 80011.

29 CFR 1910.1053(d)(1): The employer did not assess the exposure of each employee who was or may reasonably be expected to be exposed to respirable crystalline silica at or above the action level in accordance with either the performance option in paragraph (d)(2) or the scheduled monitoring option in paragraph (d)(3) of this section:  a) 912 Salida W ay, Aurora CO 80011: On or about May 3, 2023, and at times prior, the employer did not perform exposure assessment of each employee who may be reasonably exposed to respirable crystalline silica above the Action Level. Employees cut stone slabs while fabricating and installing countertops.  The employer, Amazing Granite LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(k)(6), which was contained in OSHA inspection number 1589625, citation number 1, item number 1, and was affirmed as a final order on June 8, 2022, with respect to a workplace located at 912 Salida Way, Aurora, CO 80011.  The employer, Amazing Granite LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1910.1053(d)(1)), which was contained in OSHA inspect ion number 1619313, citation number 1, Item number 2 and was affirmed as a final order on October 26, 2022, with respect to a workplace located at 912 Salida Way, Aurora, CO 80011.

29 CFR 1910.1053(i)(1)(i): The employer did not make medical surveillance available at no cost to each employee, and at a reasonable time and place, for each employee who has been occupationally exposed to respirable crystalline silica at or above the action level for 30 or more days per year:  a) 912 Salida Way, Aurora CO 80011: On or about May 3, 2023, and at times prior the employer did not make medical surveillance available for employees required to prepare stone countertop slabs for installation. Employees preparing slabs were exposed to levels of respirable crystalline silica above the action level of 25 micrograms per cubic meter of air.
