29 CFR  1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) Washington Penn Wapakoneta Plant, dba Washington Penn Plastic Co., Inc., on or about April 23, 2022, at the location of 14097 Cemetery Road, Wapakoneta, OH, 45895: The employer did not develop, document or utilize procedures for the control of hazardous energy when employees were tasked with maintenance on the Beringer Screen Changer on the "K7 Line" resulting in crushing injuries to the fingers of the left hand and amputation injury to the left thumb.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) Washington Penn Wapakoneta Plant, dba Washington Penn Plastic Co., Inc. on or about April 23, 2022, at the location of 14097 Cemetery Road, Wapakoneta, OH, 45895:  The employer or employer representative failed to contact the Toledo Area OSHA Office or the 24-hour OSHA number within 24-hours after an employee received amputation injuries and was hospitalized resulting from a workplace injury/incident.  The employer later contacted the OSHA Office on April 25, 2022.
