This violation occurred on or about April 28, 2022, and times prior thereto, in the production department, where employees operating machines where the machines guards were bypassed. Employees were exposed to an amputation hazard: (a) Production Straw Department, Jumbo Steel Machinery, Straw 104 A, Model no. JS 285/H2VA, Serial no. 4806, where the cover lid opened while machine was in operation. This was observed from a video provided by the employer of the April 28, 2022, incident. (b) Production Straw Department, Jumbo Steel Machinery, Straw 104 B, Model no. JS 285/H2VA, Serial no. 4806, straw counter interlock not working, where the cover lid open while machine was in operation where an employee received an amputation to the left index finger distal knuckle on April 28, 2022. This was observed from a video provided by the employer.
