29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about April 28, 2022, employees were exposed to struck-by and entanglement hazards in the following instances:  (a) The 220V/3-Phase Hobart dough mixing bowl and auger was not guarded.  (b) The electric J.C. Ford corn grinder (240V/3-Phase, Model # CM30-16 / SS# CM 031105) was not guarded.  (c) The conveyor belt nip points and rotating shaft to the electric tamale making machine were not guarded, the machine was operated by a Dayton Industrial Motor (3450/2850 RPMs).  (d) The cutter to electric tamale machine was not guarded, the machine was operated by a Dayton Industrial Motor (3450/2850 RPMs).

On or about April 28, 2022, employees were exposed to electrical shock and burn hazards in the following instances:   (a) In the cleaning storage area the three snap switch faceplate cover was broken and exposing live electrical parts.  (b) In the dry storage area the snap switch faceplate cover was broken and exposing live electrical parts.  (c)  The snap switch located in the back room just behind the retail store, was pushed inward beyond the metal faceplate and exposing live electrical parts.
