29 CFR  1910.178(l)(6):Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  a)  On or about May 3, 2022, the employer did not ensure that employees who were authorized to operate powered industrial vehicles were certified.  The employees had not been trained and evaluated as required.  No forklift operators had been sent for training. No forklift training or certification has been conducted at the worksite since August 2020.  The employer stated one of the department supervisors would watch new employees operate forklifts to ensure they were safe. They did not document it or provide any certification.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
