29 CFR  1926.1416(e)(5)(i):The operator did not verify that the position of the outriggers or stabilizers was correct (in accordance with manufacturer procedures) before beginning operations requiring outrigger or stabilizer deployment.  a) 1120 William Flynn Highway, Glenshaw, PA, on or about May 5th, 2022 the employer did not ensure that the National Crane 1400H truck crane had the outriggers extended in accordance to manufacturers specifications thus exposing employees to tip over hazards of the crane.

29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) 1120 William Flynn Highway, Pittsburgh, PA 15116, On or about May 5th 2022, employees were not protected from fall hazards of approximately 20 feet while installing a new roof on a commercial building that was measured at a  6/12 pitch. Employees were installing a new metal roof and no fall protection was in use. i.e. harnesses, guard rails, etc.
