29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition:  (a) POUDRE VALLEY CO-OPERATIVE ASSOCIATION, INC. at 225 NW Frontage Road, Fort Collins, CO 80524: On and before May 10, 2022, the employer did not ensure that walking-working surfaces were maintained in a safe condition before employee use, in that employees were required to walk on a metal cat-walk in the Agronomy building that was known by management to have structural defects. This condition exposed employees to falls of approximately 15 feet with associated injuries including, but not limited to, lacerations, fractures, and head injuries.  (b) POUDRE VALLEY CO-OPERATIVE ASSOCIATION, INC. at 225 NW Frontage Road, Fort Collins, CO 80524: On May 10, 2022, the employer did not ensure that walking-working surfaces were maintained in a safe condition before employee use, in that an employee was required to walk on a metal cat-walk in the Agronomy building that was known by management to have structural defects. This condition led to an employee fracturing his pelvis when he fell an estimated 15 feet after a section of the metal cat-walk's flooring gave way.

29 CFR  1904.41(a)(2): Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form.  (a) POUDRE VALLEY CO-OPERATIVE ASSOCIATION, INC. at 225 NW Frontage Road, Fort Collins, CO 80524 : On and before May 11, 2022, the employer failed during calendar year 2021, to electronically submit information from their OSHA Form 300A or equivalent by 03/02/2022. The establishment employed 52 employees and was classified as  424910 - Farm Supplies Merchant Wholesalers during calendar year 2021.
