29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  On or about December 24, 2025, employees engaged in conveyor maintenance activities at the off-site dry storage campus for the facility located at 35885 Ravenna Road, Ravenna, NE 68869 were exposed to an unexpected energy release hazard in that the company did not document set lockout/tagout for conveyor belt maintenance. These activities include but are not limited to:   i. On or about December 10, 2025, two employees were engaged in conveyor belt maintenance activities of an Undercar Conveyor: Manufacturer: R&R Manufacturing Serial Number: 003546903  ii. On or about December 10, 2025, two employees were engaged in conveyor belt maintenance activities of a 12-inch Tri Roll Conveyor: Manufacturer: R&R Manufacturing Serial Number: 00380310-07.  iii. On or about December 10, 2025, two employees were engaged in conveyor belt maintenance activities of a 36-inch Tri Roll Conveyor: Manufacturer: R&R Manufacturing Serial Number: 0038955-09.  iv. On or about December 10, 2025, two employees were engaged in conveyor belt maintenance activities of a 56-inch Conveyor: Manufacturer: Unreadable: Serial Number: P/N 12008700.
