29 CFR  1926.502(d)(6)(iii):A snap hook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snap hook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snap hook such that unintentional disengagement could occur by the connected object being able to depress the snap hook keeper and release itself.  (a) Location - 640 Main Street, Haverhill, MA  On or about 05/16/2022, employees had connected to one anchor with two lifelines exceeding the 5000 lbs safety factor

Where manufacturer's specifications are not available, the limitations assigned to the equipment shall be based on the determinations of a professional engineer competent in the field. 29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  Location -  640 Main Street, Haverhill, MA  On or about 05/16/2022, employees were observed on two roofs with no personal fall protection working at heights greater than 6 feet.  The LU Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard29CFR1926.501(b)(13), which was contained in OSHA inspection number 1474211, citation number 01, item number 01 and was affirmed as a final order on06/17/2020, with respect to a workplace located at 10, 12, 15, & 17 Hearth Lane, Bath Maine.
