29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: 2 Southwick St, Mendon, MA: On May 20, 2022 and at times prior, the employer had not established an energy control program to protect employees and outside contractors who performed maintenance and servicing activities on machinery and company vehicles. Abatement Notes: Energy control procedures are required to be: - Documented for all multiple-energy source machines including the Skyline Safari ride - Followed by vehicle repair employees including any procedures from vehicle manufacturers Energy Control training is to be documented and is required for all employees who are, or may be, in an area where energy control procedures may be utilized. A periodic inspection shall include the following elements: - Appointment of an authorized employee to inspect each local procedure for inaccuracies during an actual energy control task - A representative sampling of inspections of technicians utilizing vehicle manufacturer's procedures - A review by inspector with each authorized employee, of that employee's responsibilities under each energy control procedure they are authorized to use - Certification that the periodic inspections have been performed by identifying the machines or equipment on which the energy control procedures were being utilized, the dates of the inspections, the employees included in the inspections, and the persons performing the inspections

29 CFR 1904.41(a)(2):29 CFR 1904.41(a)(2) Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee.
