29 CFR  1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:  (a) Worksite- On or about May 25, 2022,  employees were exposed to fall hazards when working from a "6 feet- Baker" mobile scaffold.  The wheels of the scaffold were not locked to prevent movement of the scaffold while it was used in a stationary manner.  Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   (a) Worksite- On or about May 25, 2022,   a "6 feet- Baker" mobile scaffold was utilized by employees who were not trained on the applicable standards or procedures to be followed to minimize scaffolding hazards.  Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
