29 CFR  1926.102(a)(1):  The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  On or about May 2, 2022 the employer William Mullet & Sons at jobsite located at 1260 State Route 250 Norwalk OH 44807, did not ensure that each affected employee used appropriate eye or face protection when preforming roofing and framing work on a new agricultural structure while exposed to potential struck by hazards (flying particles/debris) while using pneumatic nail guns.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety nets:  On or about May 2, 2022 the employer William Mullet & Sons at jobsite located at 1260 State Route 250 Norwalk OH 44807, did not ensure employees were protected from falls while performing roofing activities atop a low sloped agricultural pole built structure. Two employees preparing the trusses of the structure for metal roofing material were exposed to a 14 feet fall on a 2-12 pitch roof to the ground below.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about May 2, 2022 the employer William Mullet & Sons at jobsite located at 1260 State Route 250 Norwalk OH 44807, did not ensure the side rails of one portable aluminum extension ladder was extended at least three feet above the landing surface. Two employees were exposed to falls from approximately 14 feet or greater from the top of the second (roof) level to the gravel decking below while using the ladder to access the second (roof) level to perform framing and roofing activities on an agricultural pole building.
