29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  ELECTRIC SHOP: The Wilton 20-inch floor mount drill press, model 2550, SN 7090012, was not  guarded to protect employees from contact with the rotating bit, exposing them to caught-in hazards.

29 CFR  1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  ENVIRONMENTAL LABORATORY: Two receptacle boxes with knockouts, designed for permanent mounting, were provided with a flexible cord, exposing employees to electrical and fire hazards. The receptacle boxes and flexible cord was used as power taps to provide electrical power to an Environmental Express HotBlock, an automatic timer, and a Milwaukee battery charger.

29 CFR  1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v)  FIRE ASSAY ROOM: An employee who worked with nitric acid was exposed to hazards from an unlabeled container. Nitric acid is both a corrosive and an oxidizer.
