29 CFR  1926.1428(a): The employer of the signal person did not ensure that each signal person meets the Qualification Requirements (paragraph (c) of this section) prior to giving any signals.  990 North Pleasant St. Amherst, MA 01002: On 6/16/2022, and at times prior, the employer had not trained their employees to meet the signal person qualification requirements.   Training shall include topics such as but not limited to arm and hand signals, capabilities of the equipment in use and ensuring that the lift area is clear of employees or other obstacles before signaling the crane operator to proceed with the lift. On 5/12/2022, an employee working on top of a residential modular housing unit and was exposed to a crushed-by hazard after giving an all-clear signal to the crane operator and not ensuring other employees and himself were clear of the lift area.
