29 CFR  1904.41(a)(2):  The employer had not electronically submitted OSHA Form 300A Summary of Work-Related Injuries and Illnesses when establishment with 20 or more employees but fewer than 250 employees in designated industries to OSHA designee when were required under this part once and year and no later than the date March 2, 2022:  a)  In the Loyalhanna Care Center located at 535 McFarland Rd. Latrobe, PA, on or about June 8, 2022 - the employer failed during calendar year 2022, to electronically submit information from their OSHA Form 300A or equivalent by March 2, 2022.

29 CFR  1910.502(q)(2)(ii)(A): The COVID-19 log did not contain, the employee's name, one form of contact information, occupation, location where the employee worked, the date of the employee's last day at the workplace, the date of the positive test for, or diagnosis of, COVID-19, and the date the employee first had one or more COVID-19 symptoms, if any were experienced:  a) In the Loyalhanna Care Center located at 535 McFarland Road, Latrobe PA, on or about June 8, 2022 - the employer failed to ensure the COVID-19 log contained employees contact information, occupation,  the date of the employee's last day at the worksite and the date the employee first symptoms developed.
