29 CFR  1910.179(n)(2)(iii):The employer did not ensure that care was taken to prevent the sling from contacting obstacles.  a) Yoder Machinery Sales Inc, located at 1500 Holloway Rd. Holland, OH 43528: On or about June 2, 2022, the employer failed to ensure employees tasked with operating overhead cranes with attached slings avoided contacting obstacles such as but not limited to an elevated Genie man-lift in use by an employee. An employee was exposed to a fall of approximatly12 ft. after a 14ft chain sling leg attached to a traveling overhead crane contacted the man-lift.

Holland, OH 43528: On or about June 13, 2022, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar years 2022. Most recently June 2, 2022, unrecorded hospitalization: Employee working upon elevated platform that was knocked over by overhead crane, resulted in fractured vertebrae, cracked ribs and a dislocated thumb. This employee was admitted to the hospital for in-patient treatment for injuries that occurred.
