29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: (a) On and preceding June 22, 2022, at Lot 93, 10398 Tipperary Drive, Plain City, OH 43064, the crew employed by KTEB was working underneath and adjacent to a roof gable suspended from a rough-terrain forklift, in areas with possible danger of head injury from impact or falling object, without protective helmets. The load was not properly secured and balanced, and the forklift operator drove the forklift forward while lifting the load while employees guided the gable into place.
