29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: At the jobsites located at 27118 SW 17th St Topeka, Kansas, employees engaged in residential framing activities were exposed to falls from approximately 10 feet from the roof to the ground below, in that the aluminum ladder used to access the roof did not extend at least 3 feet above the eave of the roof.

29 CFR 1926.416(e)(1):Worn or frayed electric cords or cables were used: Employees were exposed to electrical shock hazards. At the jobsites located at 27118 SW 17th St Topeka, Kansas, employees engaged in roofing activities were exposed to electrical hazards in that the extension cord used to supply power to the circular saw was frayed. The insulation had frayed exposing the conductors.
