29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.   a-558 E Crescent Ave, Upper Saddle River NJ 07458: (Unprotected elevated deck) Employees engaged in roofing activities on an elevated ledge were exposed to fall hazards of approximately 19ft to the asphalt below in which a portable extension ladder whose side rails were not raised to 3ft above the upper landing surface. Condition observed on or about 07/06/2022

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a-558 E Crescent Ave, Upper Saddle River NJ 07458: (Unprotected elevated deck) Employees engaged in roofing activities on an elevated ledge were exposed to fall hazards of approximately 19ft to the asphalt below in which a means of fall protection had not been utilized.
