Indiana St. Edon, OH: The employer did not require personal protective equipment to be worn such as but not limited to face shields, gloves, and heat sleeves when working near pressurized 500 Degree molten plastic of the Cincinnati Plastic injection machine 13. Employees tasked with removing blocked nozzles on machines that weren't locked out were exposed to burn hazards.

29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Plas-Tec Corp. on or about July 13, 2022, at the address of 601 W. Indiana St. Edon, OH: The employer's energy control procedure for #13 plastic injection machine, failed to identify and include procedures to isolate the buildup of pressure created by the heating of plastic within the heat barrel. Employees performing maintenance/servicing activities such as but not limited to clearing a blocked injection nozzle, without the benefit of energy control, were exposed to burns from pressurized molten plastic.

Plas-Tec Corp. on or about July 13, 2022, at the address of 601 W. Indiana St. Edon, OH: The employer did not ensure periodic audits of the energy control procedures were completed at least annually. Employees are required to perform mold change activities on machines including but not limited to the #13 plastic injection machine which requires energy control procedures that have not been reviewed for deficiencies. In this condition employees were exposed to burn hazards.
