29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  On or about July 26, 2022 the employer Yeison Cruz-Leon DBA Alonso Corporation at jobsite located at 10380 Fremont Pike, Perrysburg, OH 43551, did not ensure that each affected employee used appropriate eye or face protection when performing commercial carpentry and framing work on a new commercial restaurant structure while exposed to potential struck by hazards (flying particles/debris) while using pneumatic nail guns.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about July 26, 2022 the employer Yeison Cruz-Leon DBA Alonso Corporation at jobsite located at 10380 Fremont Pike, Perrysburg, OH 43551, did not ensure employees were protected from falls while performing roofing activities atop a low sloped commercial wood lumber framed structure. Six employees preparing for and installing trusses on the commercial wood framed building were exposed to a 11 feet or greater fall to the ground below.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about July 26, 2022 the employer Yeison Cruz-Leon DBA Alonso Corporation at jobsite located at 10380 Fremont Pike, Perrysburg, OH 43551, did not ensure the side rails of one portable aluminum extension ladder was extended at least three feet above the landing surface. Six employees were exposed to falls from approximately 11 feet or greater from the top of the second level to the gravel and concrete decking below while using the ladder to access the second (roof) level to perform framing and truss installation.
