29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about July 26, 2022:  a. In the machine shop area, employees were exposed to caught-by and struck-by hazards while using a Bridgeport mill without a guard.  b. In the machine shop area, employees were exposed to caught-by and struck-by hazards while using a Atrump mill without guard.

On or about July 27, 2022, the employer did not electronically submit information from their OSHA Form 300A or equivalent by March 2, 2022.
