29 CFR  1926.1053(b)(4): Ladders were used for purposes other than what they were designed for:    a) Jobsite, 1144 Kirkbride Street, Pittsburgh, PA: On or about August 3, 2022,  during electrical work activities, the employer did not ensure that a step ladder was used in accordance with it's intended design in that an employee used a rear brace as a step while he straddled over the top of the ladder to do work.

29 CFR  1926.1053(b)(13): The top or top step of a stepladder was permitted to be used as a step:     a) Jobsite, 1144 Kirkbride Street, Pittsburgh, PA: On or about August 3, 2022, during electrical work activities, the employer did not ensure that a ladder was used safely in that an employee stood on the top step of a step ladder while straddling the very top of the ladder as he worked.

29 CFR  1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after any occurrence that could affect their safe use:   a) Jobsite, 1144 Kirkbride Street, Pittsburgh, PA: On or about August 3, 2022, during electrical work activities, the employer did not ensure that a step ladder was appropriately inspected for defects in that many of the ladder's stickers/labels were either faded, torn off or in some way illegible.
