The pothole also created hazards to employees operating forklifts in the area.  b) Jobsite, 363 Station Road, Scenery Hill, PA 15360:  Observed on or about August 8, 2022, a large pothole was observed on the floor of the ramp floor located near the exhaust fan and emergency eyewash station.  The pothole crated a trip hazard to employees working in the area.  The pothole also created hazards to employees operating forklifts in the area.  c) Jobsite, 363 Station Road, Scenery Hill, PA 15360:  Observed on or about December 8, 2022, a pothole was observed on the floor near right side of the scale on the lower level.  The pothole created a trip hazard to employees working in the area.  The pothole also created hazards to employees operating forklifts in the area.  d) Jobsite, 363 Station Road, Scenery Hill, PA 15360:  Observed on or about December 8, 2022, a pothole was observed on the floor of the entrance way to the lower level scale area from an adjacent room near the bottom of the ramp.  The pothole created a trip hazard to employees working in the area.  The pothole created hazards to employees operating forklifts in the area.

The forklift data plate listed the lift capacity as 7650 pounds (at a load height of 173.8 inches) when using 60-inch forks. The employer installed 84-inch forks on the forklift but did not ensure that the forklift data plate was changed to reflect the reduced lift capacity.  The lift capacity was reduced by 300 pounds to 7350 pounds.
