29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.   a) Jobsite, 30 Georgetown Road, Canonsburg, PA 15317: On or about March 2nd, 2024, an employee suffered burn injuries to multiple parts of his head, face and left arm/hand while the employee used a homemade ladle to scoop out the excess molten metal from the overfilled container (sow). Once the ladle was dipped into the molten metal, the molten metal exploded. The employee was wearing wrist length cotton gloves, blue jeans, no neck protection and only a face shield for head and face protection. The employee was not wearing additional appropriate protective equipment to prevent burns to his head, face, left arm and hand.   The company should have provided and mandated the wearing of the following personal protective equipment to the employee when he is working with molten metal:  *  Elbow length aluminized/leather gloves.  *  Nomex fire hood that would protect the head, neck and upper chest/shoulder areas. *  Aluminized jacket and pants. *  Aluminized sleeves. *  Moulders steel toe/metatarsal boots. *  Hard hat.
