29 CFR  1926.352(i): Drums containers, or hollow structures which have contained toxic or flammable substances were not, before welding, cutting, or heating was undertaken on them, either be filled with water or thoroughly cleaned of such substances and ventilated and tested.   On or about August 4, 2022 at this work place, employees conducted welding and cutting operations on a petroleum tank/vessel door sheet,  the vessel was not filled with water, nor was it cleaned, ventilated or tested adequately to prevent flammable substances from ignition and exploding.

29 CFR  1926.352(j): Before heat is applied to a drum, container, or hollow structure, a vent or opening was not provided for the release of any built-up pressure during the application of heat.   On or about August 4, 2022 at this work place, employees conducted welding and cutting operations on a petroleum tank/vessel door sheet,  the door sheet was not removed from the tank/vessel opening to provide for the release of any build-up pressure during the application of heat.
