On or about  August 8, 2022, at the left rear corner of the house, the employer exposed an employee to an eye injury hazard in that the employee was not provided protective eye glasses while cutting OSB sheeting board with the DeWalt Circular saw.  b.  At 59 Banyan Forest Drive, Saint Augustine Florida.  On or about  August 8, 2022, on the roof, the employer exposed two employees installing roof sheathing boards to an eye injury hazard in that the employee were not provided protective eye glasses while operating pneumatic nail guns

29 CFR  1926.416(e)(1): Worn or frayed electric cords or cables were used:  a.   At 59 Banyan Forest Drive, Saint Augustine Florida.  On or about  August 8, 2022, at the left rear corner of the house, the employer exposed an employee to a 115-Volt electrical shock hazard in that the 12-gauge 50-foot extension cord had torn and damaged insulator covering with exposed conductors when being used to operate the DeWalt Circular Saw.

At 59 Banyan Forest Drive, Saint Augustine Florida.  On or about  August 8, 2022, on the steep roof, the employer exposed three framing employees to 9-foot to 16-foot fall hazards from unprotected edges of the roof sheathing in that they were not protected by a fall protection system while installing OSB roof sheathing boards.

On or about  August 8, 2022, at the front of the house, the employer exposed three framing employee who accessed the roof to a 10-foott fall hazard in that the roof access ladder side rails did not extend at least three feet above the roof eave.

On or about  August 8, 2022, at the front of the house, the employer exposed a framing employee to a 4-foot fall hazard in that the employee used a folded stepladder that was leaned against the side of the house, a practice not approved for that type of ladder.  b.   At 59 Banyan Forest Drive, Saint Augustine Florida.  On or about  August 8, 2022, on the front of the house, the employer exposed three employees to  a 10-foot fall hazard in that the roof access ladder was the fly section from a two-part extension ladder leaned against the side of the house, a practice not approved for that type of ladder.

On or about  August 8, 2022, on the front of the house, the employer exposed three employees to  a 10-foot fall hazard in that the fly section of an aluminum Louisville extension ladder used for roof access had visible damage that had not been repaired and the ladder had not been removed service.
