29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.) On or about August 12, 2022, at the clubhouse,  employees in the process of installing tiles on the roof and conducting cleaning activities  on a  6 in 12 slope (vertical to horizontal) roof, were exposed to fall hazards up to 9'5", in that, fall protection equipment was not installed or utilized.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     a.) On or about August 12, 2022, at the club house, employees were exposed to a 9ft 5 in. fall hazards in that, the  ladder's side rails were not extended at least 3 feet above the upper landing surface and the ladder was not secured to the building.

29 CFR  1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.   a.) On or about August 12, 2022, at the club house, employees were exposed to a 9ft 5 in. fall hazards in that, the  ladder was set up near the doorway which could cause displacement.
