29 CFR 1926.502(d)(15)(I): Anchorages used for attachment of personal fall arrest equipment were not designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two: a) Main House- On or about August 14, 2022, an employee sustained injuries during a fall from a roof where the anchor point for their fall protection system failed. The SeamSAFE M6 standing seam metal roof anchor that the employer attached to the roof as part of a personal fall arrest system did not support at least twice the intended load. b) Main House- On or about August 14, 2022, an employee working from the roof was not protected from a fall hazard where their personal fall arrest system was not complete; their personal fall arrest system did not include a deceleration device, as required by the SeamSafe M6 standing seam metal roof anchor and the SAFEWAZE SW-EX8000 removable trailing rope grab to which they were connected. The SeamSafe M6 standing seam metal roof anchor failed when it was impacted due to the employee falling from the roof without the use of a deceleration device. c) Barn/Garage roof- On or about August 14, 2022, employees working from the roof were not protected from a fall hazard where a Guardian 2-way standing roof anchor was installed on a roof with a roof pitch greater than 4/12. The Guardian 2-way standing seam roof anchor was not rated for use on a roof with a pitch greater than 4/12.

The top of the ladder was supported against the wall at a height of 19 feet and the base of the ladder was 10 feet off of the wall. b) Site- On or about August 14, 2022, employees working from a fiberglass extension ladder were not protected from a ladder collapse hazard where the ladder was erected with a horizontal distance from the top support to the foot of the ladder greater than one quarter of the working length of the ladder. The top of the ladder was supported against the wall at a height of 21 feet and the base of the ladder was 12 feet off of the wall.
