29 CFR  1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not  protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  195 Charlton Rd., Sturbridge: On or about August 19, 2022 two employees engaged in installing the wooden trusses at this commercial property with unprotected sides and edges greater than 15 feet to the next lower level were not protected by either guard rails, safety net systems, or personal fall arrest systems,  exposing them to fall from height hazards.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR  1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails did not  extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  195 Charlton Rd., Sturbridge: On or about August 19, 2022 two employees were observed using  portable aluminum extension ladders to gain access to an upper landing surface whose side rails did not extend any distance past the upper landing surface.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR  1926.1053(b)(5)(i):Non-self-supporting ladders were used at an angle such that the horizontal distance from the top support to the foot of the ladder was  not approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):  195 Charlton Rd., Sturbridge: On or about August 19, 2022,  the employer had not ensured that two employees using  portable extension ladders to access an upper level were installed in manner in which the working angle did not exceed a ratio of 4:1 or one quarter the working length of the ladder. The ladder was set up at a six -tenths angle exposing employees to ladder displacement and fall from height hazards.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
