29 CFR  1910.151(b) Adequate first aid supplies were not readily available.  a) Main Bay: On or about July 18, 2022, Physicians First Medi-First Pain Relieving Burn Spray was stored in the first aid kit for employee emergency use, but the burn spray had expired on September 2012, exposing employees to biological hazards.  b) Main Bay: On or about July 18, 2022, SWIFT Multi symptom Cold Relief tablets was stored in the first aid kit for employee emergency use, but the tablets had expired on January 2013, exposing employees to biological hazards.  c) Main Bay: On or about July 18, 2022, Water-Jel Triple Antibiotic First Aid Ointment was stored in the first aid kit for employee emergency use, but the ointment had expired on November 2014, exposing employees to biological hazards.

29 CFR 1960.67 As required by 29 CFR 1904.32, the Agency did not certify that he or she has examined the OSHA 300 log and that the annual summary was correct and complete:  29 CFR 1904.33(a) The employer did not save the OSHA 300 Log, the privacy case list (if one existed), the annual summary, and the OSHA 301 Incident Report Forms for five (5) years following the end of the calendar year that these records covered.   a) On or about July 18, 2022, the employer did not retain at the establishment OSHA 300 logs for fiscal years 2019, 2020, 2021, and 2022.  b) On or about July 18, 2022. the employer did not retain at the establishment OSHA 300A annual summary forms for fiscal years 2019.
