29 CFR  1910.147(c)(8): Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance.  On or about September 12, 2022, and at times prior thereto, in the shop area, an unauthorized employee was exposed to electrical, struck-by and caught-by hazards while performing maintenance and servicing work on machines locked-up by another employee.

29 CFR  1910.305(j)(2)(iv): A receptacle installed in a wet or damp location was not suitable for the location.  On or about September 12, 2022, and at times prior thereto, in the plating line area, employees were exposed to electrical hazards while using hoses to spray water onto lifting baskets within inches of electrical extension cords that were plugged into non-GFCI outlets.
