29 CFR  1926.100(a):At this job site, employees exposed to possible head injury from falling or flying objects or from impact with hand tools, equipment and building materials during construction operations were not wearing hard hats.   On or about September 12, 2022, and times prior to, two employees were working on the ground and passing material, without wearing hard hats, above their heads to employees working on the roof, exposing the employees on the ground to struck-by hazards.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  On or about September 12, 2022, and times prior to, two employees were using a ladder to climb to the roof in which the side rails of the ladder did not extend three feet above the walking surface of the roof, exposing the employee to fall hazards.
