The employer failed to ensure that  the bailer's machine guarding was effective and fully functioning.

29 CFR  1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone:   a) PolyTech Fibers, LLC, production warehouse floor, bailer area: On or about September 15, 2022, and at times prior, employees were exposed to amputation hazards in that special hand tools for placing and removing material(s) were not provided at the point of operation of the baler (bailer) machines to prevent employees from having any part of their body in the danger zone the during operating cycle, at the workplace. The employer failed to provide tools for placing material and removing fibers from the point of operation on the bailer machines in line 1 and line 2.
