29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. On or about January 8, 2026, and at times thereafter, the employer did not keep all places of employment, passageways, storerooms, and walking working surfaces in a clean, orderly and sanitary condition. Accumulations of paper pieces, paper particulate, and other material were present on walking-working surfaces, walls, equipment, duct work, electrical boxes, pipes, horizontal structural I-beams, steel cross-bracings, on top of the QC testing room and other elevated surfaces. Employees were exposed to potential fire hazards from the accumulations of combustible materials.

29 CFR 1910.134(a)(2): A respirator was not provided by the employer to each employee when such equipment was necessary to protect the health of the employee: On or about February 24, 2026, the employer had not provided an employee working in the back loading area of the production line performing tasks such as mixing paper goods and loading paper goods onto the paper table with a respirator for protection against particulates not otherwise regulated- total dust. The employee was exposed to an 8-hour time weighted average (TWA) of 24.4 mg/m3 of total dust, approximately 1.63 times the permissible exposure limit (PEL) of 15 mg/m3 without the use of respiratory protection.

29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of Particulates Not Otherwise Regulated (Total Dust) listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 15 mg/m3: On or about February 24, 2026, an employee working in the back loading area of the production line was exposed to an 8-hour time weighted average (TWA) of 24.4 mg/m3 of total dust, approximately 1.63 times the permissible exposure limit (PEL) of 15 mg/m3 while performing tasks such as mixing paper goods and loading paper goods onto the paper table. The exposure was derived from a personal sample collected over a 463 minute sampling period, with zero exposure assumed for the unsampled period of 17 minutes.
