King of Prussia, PA., Roof: On or about September 8, 2022, employees used the stairways with unfilled step pans/landing to access the roof.

29 CFR  1926.1052(c)(1)(ii):Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:  (a) 880 East Swedesford Rd. King of Prussia, PA., Roof: On or about September 8, 2022, employees used a stairway that did not have a stair-rail system along each unprotected side or edge.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a) 880 East Swedesford Rd. King of Prussia, PA., Roof: On or about September 8, 2022, employees were observed sweeping along the flat roof edge of a two-story commercial building, but guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system was not in place while exposed to the 25-foot hazard.     Lopez Express Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1574479, Citation 1 Item Number 1 and was affirmed as a final order on March 03, 2022, with respect to a workplace located at 900 Route 73 North A700 Marlton, NJ 08053.
