29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. On or about September 19, 2022, at the facility located at W2874 Highway 64, Marinette, Wisconsin, the employer exposed employees to contact with electrical current by failing to ensure the Campbell Hausfeld air compressor (Model No.: SS5106V, Serial No.: 030685L-180402) junction box cover was properly installed.

On or about September 19, 2022, at the facility located at W2874 Highway 64, Marinette, Wisconsin, the employer allowed flexible cords to be used as a substitute for fixed wiring. a. An extension cord was used to supply power to the computer used at the CNC Machining Center. b. An extension cord was used to power a light fixture at the workbench for the Mitsubishi 3015 LVPlus Laser.
