The employer required the employees to bring their own personal fall arrest equipment, and did not provide the equipment at no cost to the employees.

Location: a) Supported scaffold on the north side of the building:  On or about September 20, 2022, employees installed a winch hoist, for material hoisting, on the top tier platform of the scaffold. Subsequently, the employees removed two planks from the scaffold platform underneath the hoist, to provide an opening to hoist material.  The employer did not have a competent person that inspect the scaffold before the work shift and after the hoist was installed and the planks were removed.

Location: a) Supported scaffold on the north side of the building:  On or about September 20, 2022, employees installed a winch hoist, for material hoisting, on the top tier platform of the scaffold. Subsequently, the employees removed two planks from the scaffold platform underneath the hoist, to provide an opening to hoist material.  The employees that installed the hoist and removed the planks were not trained in scaffold erection, moving, dismantling or alteration. There was no competent person to supervise and direct employees in scaffold alteration.
